"Last week, one of their drivers was having an exceptionally bad day, and just decided that they weren't going to deliver about 8 patient test result envelopes, and tossed them in a garbage can at the gas station. The gas station attendant found the envelopes and called the laboratory to express their concern and alert them of their discovery."

"One of the drivers was asked by one of the clinics to transport a bag of dirty needles back to the main office for disposal. Not knowing any better, the driver agreed, and consequently suffered a needlestick injury from a needle that punctured the bag."

Dry Ice: Anyone can use it. Or can they?

"I got a call late last week from a courier in the Southwest who had been transporting dry ice in a large ziplock bag in a cooler in the back seat of his car, when he heard a hissing noise and pulled over. The nylon cooler he had put the bag of dry ice in had expanded and was stretched nearly to the breaking point"

One company was cited and fined for not having their chains secured in the cab of the company truck and for not having a MSDS sheet available for the detergent they used to wash company vehicles."

There's life (and death) in the blood...

"As I was coming up from the basement, I tripped on the 3rd stair from the landing and several blood specimens shot out of the envelope and broke on the landing. Now there's a bloody mess! Imagine if I had had no training on using a spill kit, or how to keep others safe from a spill site or not to leave the spill site un-attended. An elderly person or child could have walked into that mess and been exposed and possibly infected."
